A high-frequency linear probe provides the best resolution. First, find illeocecal junction at the right lower quadrant by first identifying the thick cecum, directly inferior to the beginning of the ascending colon.Then, turn the probe to a sagittal orientation and move medially to visualizethe ileum. The appendix is a small tubular structure, which is acontinuation of the lower end of cecum.
Once the appendix is located, the clinician can measure its diameter. The normal caliber of the appendix in AP measurement should be less than 6 mm.
(Interestingly,in the above image, appendicitis was incidentally seen by an endovaginalprobe.)
Overall,the use of ultrasound for potential appendicitis should be promoted, as it is a safer and less expensive modality.
