我們都知道在美國或加拿大的大學中，本科不開設醫學院，高中生畢業進入大學以後是不能直接學醫的，你必須首先獲得一個本科學位，然後再去申請醫學院。
所以高中畢業生若有意向學醫，就只能在本科階段考慮Pre-Med了。不過Pre-Med事實上並不是一個專業，而是在申請醫學院前一系列必修課程的總稱。那麼這些大學課程包括哪些呢？一般是一年英文，一年微積分，一年基礎化學（有實驗經歷），一年有機化學（有實驗經歷），一年基礎生物（有實驗經歷），一年基礎物理（有實驗經歷）。也就是說只要在申請醫學院前修完了這些課程並且成績優異（當然儘量全A啦），就具備申請條件之一。（其他申請條件還包括MCAT（Medical College Admission Test）的成績以及在大學內的課外活動與醫院實習經歷。）
來看維基百科對Pre-Med的定義：
Pre-medical (often referred to as pre-med) is an educational track that undergraduate students in the United States and Canada pursue prior to becoming medical students. It involves activities that prepare a student for medical school, such as pre-med coursework, volunteer activities, clinical experience, research, and the application process. Some pre-med programs providing broad preparation are referred to as 「pre-professional」 and may simultaneously prepare students for entry into a variety of first professional degree or graduate school programs that require similar prerequisites (such as medical, veterinary, or pharmacy schools).
美國正式四年大學和二年制社區學院每年新生約兩百萬，其中會有近1/5到1/4會被大致歸類於Pre-med。如UCLA，Johns-Hopkins，Washington University of St. Louis，Duke這種傳統Premed名校甚至會高達1/4。每年有四十萬的美國大一新生打算行醫會是個不離譜的估算。其中Pre-Dental，Pre-Pharmacy都會被歸類於Premed。
美國醫學院分Allopathic和Osteopathic兩大系統，前者頒的是MD，也是一般人所知道的醫學院，有近150所。後者頒的是OD，人數較少，有三十幾所，學校比較好進，是進不了Allopathic Schools的選項之一。但在美國的醫療體系中是二等公民，際遇不如MD，但比三四等公民(外國來的醫生或外國醫學院畢業的美國學生)好。過去十年來每年美國Allopathic醫學院申請人平均五萬人不到，很穩定，錄取名額17000。Osteopathic醫學院申請人有一萬，錄取名額為4000。
牙醫學院花樣少點，美國有六十幾所，每年有兩萬人申，錄一萬人。藥學院有115所，每年也有了三萬人申請，錄取人在一萬五到兩萬間。
若加上身份因素，讀醫難度會大幅增加，MD的17000個名額中有大約300~400會是外國學生，OD則會超過一千外國學生。
這意味著每年四十萬的初生之犢若不算重覆多次申請的回鍋申請人，會有近三十五萬進不了醫學院，牙醫或藥學院。先不算完全脫離本行的人，這些申請人的最主要的出路是：
1) 醫療體系(護士，醫療助理，物理治療師，醫療保險，醫院行政管理等不需要博士學位的工作)
2) 繼續讀研，出路在科研或教書。大致可分基礎或應用醫學，基礎醫學出路會比較專比較窄，應用醫學稍廣，但這兩者待遇會高於只有BS或BA。也很有機會和同校醫學院的研究課題合作。若研究課題極尖端，極可能出人頭地。除了不能臨床看診，和專門從事研究工作的醫學院畢業生沒有分別。臺灣香港或中國醫學院畢業生到美國改走研究的人很多都是走這條路取得成就。
3) Bio-Chem, Bio-Med (Engineering)， Molecular/Cell/Micro Biology，Immunology，Physiology，Neurology等等就業目標極廣，可進生物科技，醫療儀器，製藥，生化產品，化工等一切有關的領域。
這三者加起來會包括大部份人，但華爾街和大公司對Ivy League或超級名校成績好的的理工人才或Pre-med很有興趣，因為這些人聰明又有紀律，不少華爾街或世界500強的中堅幹部都是pre-med出身。
也就是說即使讀不了醫學院，你讀Biomedical Science的出路極多，不必擔心。
最後提醒一下：對於國際學生來說，申請美國的醫學院以及將來在美國從醫是非常難的，需要極大的毅力和耐心，以及優異的成績。
