What is your diagnosis?
The answer is Duret Hemorrhage
Duret hemorrhage is a small hemorrhage in the midbrain or pons caused by the compression of severe transtentorial herniaton.
The typical CT imaging findings are small hemorrhages in the midline near the pontomesencephalic junction (A and B, arrows) and the presence of supratentorial injuries, such as contusion or subdural haematoma.
In this case, the herniation was caused by bilateral frontal contusional hemorrhages (C, arrows).
