A 52-year-old man with left-sided hearing loss and tinnitus after recent trauma

What is your diagnosis?

The answer is Perilymphatic Fistula

A transverse otic-violating temporal bone fracture (white arrow) extending across the vestibule, with pneumolabyrinth in the vestibule (yellow arrow), is in keeping with the diagnosis of perilymphatic fistula.
Perilymphatic fistula is due to abnormal communication between the middle and inner ears.
It can be posttraumatic, postoperative, secondary to cholesteatoma, or idiopathic.
Patients present with sensorineural hearing loss, tinnitus, vertigo, and recurrent meningitis.

耳部外力可使顳骨發生橫向性骨折(白色箭頭),並貫穿前庭,致使前庭內出現氣體(黃色箭頭),提示外淋巴瘻的診斷。
外淋巴瘻是因內耳與中耳出現異常交通所致。
可發生於創傷後、手術後、繼發於膽脂瘤或原發性。
患者表現為感音神經性聽覺喪失,耳鳴,眩暈以及復發性腦膜炎。
