Basilar artery occlusion accounts for 10% of unexplained, nontraumatic coma.
The mortality rate is high, around 80-90% without treatment.
Noncontrast CT often demonstrates hyperdense basilar artery (A, arrow).
CT angiography may confirm the diagnosis by showing the occlusion of the basilar artery (B, arrows).
X-ray angiography can not only validate the CT findings (C, arrow), but also allow for the recanalization of the occluded basilar artery by direct thrombectomy (D, arrow).
This patient had a complete recovery of symptoms with no neurologic deficit.
