This 25-year-old physician complained of mild visual blurring. Visual acuity in the right eye was 20/20 and in the left eye was 20/15. Note the multiple slightly elevated lesions scattered throughout the posterior pole of both eyes (A and B). Clinically, these lesions resemble somewhat a serous detachment of the retinal pigment epithelium (RPE). Note the ill-defined borders of the yellowish material in the large central lesions. Late angiogram (C) in the left eye revealed no evidence of staining of the large central lesion. The slight fluorescence of the paracentral lesions was probably caused by pseudofluorescence. The patient’s electrooculographic findings were definitely abnormal (1.1 right eye and 1.3 left eye). Several months after these photographs, he became asymptomatic and remained so for approximately 1 year, when the symptoms returned for approximately 6 months. He again became relatively asymptomatic. Photograph of the left fundus taken 21 months after A and B shows that the central lesion has largely resolved (D). The other lesions appear smaller, more discrete, and yellow in appearance. Similar findings were present in the right eye. His visual acuity at this time was 20/30 in the right eye and 20/15 in the left eye. Angiography (E) showed evidence of some thinning of the RPE inferior to the foveal area. The paracentral lesions appeared nonfluorescent.
